Information for Death Certificate


A Certified Death Certificate is necessary before anything can be done.  A death certificate can be obtained through a Funeral Director.  It is a good idea to obtain multiple copies of a death certificate as most agencies require a certified certificate and not a photocopy. This is what the funeral director needs for the certificate.

Information Required to Obtain a Death Certificate

  • Name
  • Date of birth & place of birth
  • Armed Service (years, branch,  Service #  copy of DD214)
  • Race
  • Hispanic orgin (if yes specify)
  • Number years of schooling
  • Social Security number

  • Martial status
  • Surviving spouse (maiden name if wife)
  • Usual occupation
  • Kind of business
  • Name of business
  • Residence of decedent
  • Father's name
  • Mother's name
